Telemonitoring allows SUN Home Health and Hospice nurses to more frequently monitor patients whose diagnosis -- including Heart Failure, Diabetes, Hypertension, Chronic Obstructive Pulmonary Disease, and Depression -- may require increased vital sign assessments.  This simple to use monitoring equipment guides the patient through the process of taking their vital signs and answering questions about their health condition; and then transmits that data to a central station at SUN Home Health and Hospice where a registered nurse can review, assess, and make changes and adjustments in the patient care plan. 

The telemonitoring equipment helps to provide more accurate and timely patient assessments with the goal of reducing acute care hospital readmissions for patients with chronic conditions.  The technology is used in conjunction with regular home healthcare visits by registered and licensed nurses and provides an added level of monitoring for homebound patients who have difficulty accessing other services in rural areas.
